SQRH HEALTH & WELLNESS CLINIC TOOWOOMBA

SQRH have established a student-led Health and Wellness Clinic (HaWC) in Toowoomba, located at the Baillie Henderson Hospital within the Health and Wellness Centre.

The clinic provides an interprofessional health and wellness program delivered by nursing and allied health students supervised by experienced health professionals.

Chronic disease issues are prevalent in Queensland and in particular across Southern Queensland. The Health and Wellness Clinic supports individuals with low to rising risk of chronic disease with a program to build the knowledge and skills for participants to live their healthiest life.

What does the program involve?

  • Goal setting: The clinic supports participants to set achievable goals and work towards them through the program and beyond.
  • Duration: 4 months.
  • Time: The clinic is open Monday, Wednesday and Friday.
  • Activities: Exercise programs (pool and gym), health education sessions, one-one and group consultations with our friendly students and professional staff, healthy eating and cooking skills sessions, guided sessions about local community resources.

HOW TO APPLY

Please visit your General Practitioner (GP) for a referral to our clinic. Your GP will send your referral to us via Medical Objects or email. Our contact details are listed below, as well as a referral form for your GP to complete.

Participant Information

Criteria

To join the program, a referral from a GP is required. Please discuss with your GP if you are interested in joining us.

We have a few criteria to meet to access our free program.

Inclusion criteria (meet at least two of the following):

  • BMI > 25
  • Elevated blood pressure
  • Elevated cholesterol
  • Elevated blood glucose levels
  • Poor physical activity levels (less than 150 minutes of exercise per week)
  • Poor eating habits (less than two fruit and five vegetables per day)

Exclusion criteria (if you meet any of the following then you won’t be able to access the program):

  • Acute symptomatology (physical or psychological)
  • BP greater than 200/110
  • Significant cognitive impairment
  • Pregnancy
  • Under 16 years of age
  • Active Court Proceedings related to mental or physical health and wellness
  • Access to treatment via insurance claims

There is no cost to participate.
